Please provide the following information:

Please follow these steps to send your iPaq to Kinesi Systems:

1. Backup or syncronize your iPaq with a PC or Laptop. For Expansion Sleeves remove the card devices;

2. Print this page and fill out the information at the right;
3. Include a completed copy in the package;

4. Include a Cheque or Money Order in the amount shown for the work requested;

i.e. To upgrade a single iPaq with PGF Pro Battery and HyperDrive and an expansion sleeve with a PGF Pro Battery will cost:
$223.63 + 188.32 = $411.95.
